Passports issued for 5 yrs validity period. Following documents/papers are required to be produced:
• Birth certificate along with a photocopy.
• Marriage certificate with a photocopy.
• Current passports of parents along with photocopies.
• Four recent colored passport size photos of the child.
• Affidavit to be signed by the parents - Affidavit Form.
• Duly filled in form No: EAP-I. Either of the parents can sign the application.
